Title: Full Time Clinical Case Manager
Principal duties:
- Work with youth and young adults
- Administer initial clinical assessments, assign diagnoses using the DSM IV and develop a treatment plan. Assessment should also note behavioral assets and strengths.
- Provide individual counseling to enable client to cope with current and future challenges without reliance on impaired behavior and/or decrease when appropriate.
- Provide group counseling to clients with similar issues covering various topics (ex. substance abuse, domestic violence, eating disorders, etc.)
- Complete all documentation, assessments and reports in a timely manner.
- Track outcomes and goals.
- Provide written and oral communication for client notification and follow up.
- Coordinate and assemble reports as needed.
- Assist with facilitating client orientation and program components.
- Provide case management services to the clients.
- Conduct home visits.
Desired Qualifications:
- LPC, LCSW or related license preferred.
- Must be autonomously mobile in accessing a variety of neighborhood-based facilities where meetings and programs are held.
- Experience working with underserved populations
- Experience conducting individual and group therapy
- Experience working with youth and young adults
- Must be energetic and dependable
Knowledge, Skills, and Abilities:
- Must have the ability to maintain client confidentiality, Ability to diagnosis using the DSM IV and develop appropriate Treatment Plans
- Must have a chauffeur’s license or obtain one soon after employment.
