SUPERVISOR:  Senior Manager of Clinical Case Management

 

DEFINITION AND PRINCIPLE DUTIES:

The Community Outreach Worker is active in the community and provides outreach services to a targeted population to improve their educational, vocational and employment attainment.  Outreach worker also interacts and maintains a good working relationship with program participants and their families, other program staff, and community leaders and partners. 

 

·       Actively recruits participants into the program by making initial contact with targeted clients

·       Contacts clients by phone and by actively conducting home visits

·       Explains and promotes the program to potential client referral sources and outside community agencies

·       Works on a team with other staff to develop and help client achieve goals of the his/her individual treatment plan

·       Transport the client to various places in the community in efforts to help the client reach goals listed on the treatment plan

·       Complete all documentation in a timely manner

·       Other duties as assigned

 

 

DESIRABLE QUALIFICATIONS:

 

·       Bachelor’s degree in social services, human services, or related field preferred.

·       Ability to gain and hold the support, confidence, and enthusiasm of program participants and community leaders

·       Good written/verbal communication skills and organizational ability

·       Must be articulate and capable of presenting complex concepts in simplistic terms

·       Must be attentive to administrative details regarding accurate daily reporting of attendance data, programming, and work performed

·       Must represent Provident within the community and attend community meetings

·       Must be skillful in adjusting to client needs and issues

·       Must have an interest in social services and community action

·       Experience working with low to very low income clients

·       Must have valid driver’s license, valid car insurance and a good driving record

 

PHYSICAL DEMANDS:

 

·       Requires the ability to speak, see, and hear

·       The capacity to move frequently about the office and surrounding community

·       Occasional lifting up to 10 pounds

·       Requires the ability to handle unexpected situations arising from administrative or operational issues

MENTAL DEMANDS:

  • No unusual demands
  • Must be autonomously mobile in accessing client homes and a variety of different neighborhood based facilities where meetings and programs or held.

WORKING CONDITIONS:

  •  Well-lit office with good heating, cooling, and ventilation system
  •  Handicapped accessible
  •  Good office equipment
