TITLE:      Senior Manager of Clinical Case Management

SUPERVISOR:            Vice President of Community Services     SALARY GRADE: 

DEFINITION:          

Plans, coordinates and implements clinical case management programs to youth and adult populations with multiple barriers to employment. Plans and coordinates approaches to service delivery that incorporates mental health services, advocacy, outreach and referral. Develops collaborative partnerships with community stakeholders that service similar populations.

Through the processes and duties described below, the Senior Manager of Clinical Case Management will be focused on creating positive and measurable change in clients that have multiple barriers to self-sufficiency. The Senior Manager must plan and coordinate case management strategies that address mental health, physical health, housing, transportation, education, employment, family management, and concrete needs. The successful candidate has a strong desire to work with high risk populations and seeks strategies to change behavior. The successful candidate will also thrive in an environment of collaboration and community work.

DISTINGUISHING CHARACTERISTICS:

The Senior Manager is knowledgeable in best practices for improving employment outcomes with low-income youth and adults with barriers to employment. Has experience providing community mental health, social work or case management. Knowledgeable about community resources and stakeholders and has management experience.

Principal Duties:

  • Plans and implements a comprehensive vision for case management that addresses the mental heath and basic needs of youth and adults and helps them obtain and maintain living wage employment
  • Implements all case management grants and contracts
  • Collaborates with other managers and programs within Community Services and Provident overall
  • Recruits, trains and supervises staff of case managers, trainers and job developers
  • Attends community meetings
  • Recruits and supervises volunteers and practicum students.
  • Facilitates collaborations between Provident and collaborative partners to secure additional services for clients and leverage agency resources
  • Collects and analyzes accurate data and information about clients, outcomes, and services in database management systems
  • Writes and submits reports to funders on a monthly basis.
  • Writes and submits quarterly and annual United Way reports to supervisor
  • Assists grant writers in developing proposals

Knowledge, Skills and Abilities:

  • Able to build trust and rapport and establish working relationships with clients, direct reports, Provident staff and community partners (funders, employments, social services providers, government leaders, the philanthropic community, and business community)
  • Experience conducting individual and group therapy in community settings
  • Community outreach experience
  • Excellent project planning and organizational skills
  • Attention to detail
  • Ability to collect, organize and document data.
  • Knowledge of the community in general, community institutions, organizations and networks and common practices and attitudes of service providers.
  • Comfort in initiating and facilitating discussions about the project's focus.
  • Ability to communicate clearly, concisely and effectively both orally and in writing
  • Ability to make public presentations and market programs and services

Desirable Qualifications:

  • LCSW preferred. Masters in Counseling, Education or related field accepted.
  • Prior supervisory experience.
  • Experience and skill working with at risk youth, TANF recipients, adults released from incarceration.
  • Strong computer skills with experience with project management and database management software programs.
  • Public speaking skills

Physical Demands.

  • Will work in the field often
  • Must have a valid driver's license and access to a vehicle and have the means to travel extensively within the community.
  • Must be autonomously mobile in accessing a variety of different neighborhood based facilities where meetings and programs are held.
  • Requires the ability to speak, see and hear.
  • Requires frequent movement about the office.
  • View the computer monitor.
  • Occasional lifting or carrying up to 10 lbs.
  • Requires ability to handle unexpected problems arising from administrative or operational issues.

Mental Demands

  • Challenges of collaboration externally and internally
  • Normal managerial administrative duties;
  • No unusual demands.

Working Conditions

  • Will work primarily out in the community in conditions that are not within Provident's control.  Must be able to adapt and thrive in such environments.
  • Well-lit office with good heating, cooling, and ventilation system
  • Handicapped accessible.
  • Good office equipment.
