TITLE: Senior Manager of Clinical Case Management

PROGRAM NAME: Clinical Case Management (Transitions, SWIFT, ReConnect)

SUPERVISOR: Vice President of Community Services     SALARY GRADE: Exempt

DEFINITION: Plans, coordinates and implements clinical case management programs to youth and adult populations with multiple barriers to employment. Plans and coordinates approaches to service delivery that incorporates mental health services, advocacy, outreach and referral. Develops collaborative partnerships with community stakeholders that service similar populations.

Through the processes and duties described below, the Senior Manager of Clinical Case Management will be focused on creating positive and measurable change in clients that have multiple barriers to self-sufficiency. The Senior Manager must plan and coordinate case management strategies that address mental health, physical health, housing, transportation, education, employment, family management, and concrete needs. The successful candidate has a strong desire to work with high risk populations and seeks strategies to change behavior. The successful candidate will also thrive in an environment of collaboration and community work.

DISTINGUISHING CHARACTERISTICS: The Senior Manager is knowledgeable in best practices for improving employment outcomes with low-income youth and adults with barriers to employment. Has experience providing community mental health, social work or case management. Knowledgeable about community resources and stakeholders and has management experience.
